Christian Formation Registration

Please complete this registration form no later than September 4, 2022. Tuition is $180 per family for forms received by September 4. Tuition for registration forms received on or after September 4, 2022 is $185 per family. Payment is not required with submission of this form, but please send payment as soon as possible.
